Before you decide to file a formal complaint, please read about the Informal Complaint process.  In many cases, the PUC can help you resolve your complaint without having to go through the lengthy formal complaint process.

Motor Carrier Forms
The application process for new motor carrier applicants has been streamlined. Please discard any old application forms and instructions and replace with the new forms and instructions that have a revised date of 9/11.  Please submit one (1) original of each filing to the attention of the Secretary's Bureau; DO NOT submit copies to the Compliance Office or the Bureau of Transportation & Safety.

Report of OutageEvery jurisdictional electric utility must complete and submit a Report of Outage Form to the Commission when they experience an unscheduled service interruption event that affects 2,500 or 5%, whichever is less, of their total customers for 6 or more consecutive hours. Accident Report UCTA-8All jurisdictional electric, gas and water utilities shall complete and submit this form (or a comparable form as defined in the Code) to the Commission when they experience a reportable accident involving the facilities or operations of the public utility. Electric Choice Annual ReportLicensed Electric Generation Suppliers are required to report annual sales (kWh), generation sources, gross receipts and tax ID information. Municipal authorities are required to report jurisdictional retail sales/generation sources and gross receipts. EDCs are required to report POLR sales/generation sources and tax ID information. Report of OutageEvery jurisdictional water and wastewater utility must complete and submit a Report of Outage Form to the Commission when they experience an unscheduled service interruption event that affects 2,500 or 5%, whichever is less, of their total customers for 6 or more consecutive hours.
