The Commission established a schedule for all Local Exchange Carriers to file tracking reports so that TRS surcharge revenue collection data may be effectively monitored and evaluated.

2010 Annual Tracking and 2010 Annual Access Line Summary Reports
Must be submitted to the Commission by March 31, 2011, and March 1, 2011, respectively. All LECs should file a hard copy of the report with the Secretary of the Pennsylvania Public Utility Commission, at PO Box 3265 , Harrisburg , PA 17105-3265. An electronic copy of the report should be emailed to Christopher Hepburn at chepburn@pa.gov. Any questions regarding the reports may be directed to Mr. Hepburn at (717) 214-9115.

TRS Surcharge Remittance Form - Effective July 1, 2011 through June 30, 2012.

All local service providers are required to collect and remit the TRS surcharge revenue monthly to US Bank Institutional Trust and Custody, ATTN: Sue Massey, EX-PA-WBSP, 50 South 16th Street-20th Floor, Philadelphia, PA 19102. Please make your remittance checks payable to the Pennsylvania Relay Service Fund.
